Depression

For patients who have not responded to ≥2 antidepressant trials at therapeutic doses. High-frequency rTMS to the left DLPFC is the TGA-approved protocol, eligible for Medicare rebate under MBS items 14216–14220. Neuronavigation improves target precision and may enhance treatment outcomes.

Anxiety Disorders

Low-frequency rTMS to the right DLPFC has demonstrated anxiolytic effects across multiple RCTs, with meta-analytic support for reliable symptom reduction. Given high comorbidity rates with depression, treatment is frequently co-administered. Protocol selection is individualised to symptom profile under psychiatrist oversight.

Chronic Pain

rTMS targeting the primary motor cortex (M1) and DLPFC has demonstrated analgesic effects across neuropathic pain, fibromyalgia, and central sensitisation disorders. A 2022 RCT found 66.6% of fibromyalgia patients achieved ≥30% pain reduction with DLPFC-targeted rTMS. Protocol is individualised based on pain type and chronicity.

Post-COVID Cognitive Symptoms

Prefrontal rTMS has been shown to improve processing speed, working memory, and cognitive endurance in Long COVID presentations. Neuronavigation ensures accurate engagement of the frontal circuits involved in executive function and attentional control, which are frequently disrupted following SARS-CoV-2 infection.

Migraine

Repetitive TMS protocols targeting the occipital cortex and DLPFC have shown preliminary efficacy for both acute migraine interruption and prophylactic frequency reduction. Evidence from RCTs is encouraging but consensus on optimal protocol — target, frequency, and session number — has not yet been established. Delivered under psychiatrist and neurology-informed oversight.

Parkinson's Disease

High-frequency rTMS over the motor cortex and supplementary motor area has shown improvements in UPDRS motor scores across multiple RCTs and meta-analyses, with additional benefit for mood and sleep disturbance. Evidence quality remains low-to-moderate per GRADE assessment; treatment is offered as an adjunct to standard neurological management under specialist oversight.

Complex Regional Pain Syndrome

Motor cortex rTMS has been investigated as a neuromodulatory approach for CRPS-related central sensitisation, with early RCT data showing short-term pain reduction. Evidence remains limited in sample size and follow-up duration. Treatment is considered in patients who have not responded to conventional pain management, under multidisciplinary specialist oversight.

Motor cortex stimulation via rTMS targeting the trigeminal representation within M1 has shown analgesic effects in small RCTs and observational studies of refractory orofacial pain. Evidence base is preliminary; treatment is appropriate for patients who have exhausted pharmacological options, under specialist-supervised, neuronavigation-guided protocols.

Menopause-Related Brain Fog

Hormonal transition disrupts prefrontal circuit function, contributing to cognitive slowing, attentional difficulties, and low mood. Prefrontal rTMS targeting the DLPFC may support working memory and mental clarity in perimenopausal and postmenopausal presentations. Clinical evidence is emerging; treatment is individualised under psychiatrist oversight.
